San Mateo AYSO Soccer - Late Registration
The Open Registration period for San Mateo
AYSO is now closed.
Late Registration may only be done by mail,
and is on a first-come, first-serve basis. Priority is based on
the order the league receives the competed forms and player fees.

Start by pre-registering your children in the eAYSO registration system.
Please note that the AYSO system is down for maintenance on Monday
evenings 6 PM - 10 PM |

Follow the step by step
instructions to input all of the necessary information and print out
your registration forms. |

If you are a first
time user of eAYSO, follow the step by step instructions for a new
user...it is easy! If you used eAYSO last year
please use the username (email) and password which you used in creating
an eAYSO account. |

Note: It is possible that this could be different from
the email address you currently use. You will discover that all of the information you entered last
year is still there ... just log on and open the Myeayso windowshade. Go to
the parent section and update all of your information for both parents,
then follow the steps to update or add any players and print their
forms. |

You will need to print 2
copies of the "Player Pre-Registration Form"
(printed from eAYSO)
for each player. |
| 5. |
Players in the U16 and U19
divisions only: (14 years and older on July 31st) must include a
wallet-sized picture (e.g.: Passport photo, school photo, etc.). |

Write a check. The
Registration fee is $115 per player (to SM AYSO). This will not be
cashed until your child is placed on a team. Write ONE CHECK PER
PLAYER.

Note: Bank fees associated with bounced
checks (e.g.: closed account, not sufficient funds, etc.) must be paid
in addition to the registration fee for the player to play. |

Make a photocopy of
your child's age verification if your child is new to AYSO in San Mateo
(birth certificate, passport, insurance card - if birthdate is listed). Do
not send an original document as it will not be returned. |

Print and sign a copy of the Parent Athlete Concussion Information
Sheet available <here>.

Mail the following player registration materials to the address listed
below.
- Two copies of
the player pre-registration form.
Both copies of this form must be
signed!
- One copy of the Parent / Athelete Concussion
Information Sheet.
This form must be
signed!
- A wallet-sized picture (for players 14 and older;
those in divisions U16 and U19)
- A check for $115
(one check for each player)
- Photocopy of age
verification (if new to San Mateo AYSO)

child's name and age in your message. |

Incomplete or unsigned registration packets will be returned. |

a part of your child's soccer experience!
