San Mateo AYSO Soccer - Late Registration

The Open Registration period for San Mateo AYSO is now closed.

Late Registration may only be done by mail, and is on a first-come, first-serve basis.  Priority is based on the order the league receives the competed forms and player fees.

1. Get on the web and log on to www.eayso.org.  Problems with eAYSO should be directed to Keith Jobe our webmaster by email. 2. Follow the step by step instructions to input all of the necessary information and print out your registration forms. 3. If you are a 1st time user of eAYSO, follow the step by step instructions for a new user…it is easy! It is strongly recommended to get a user name and password to save you time next year! 4. If you used eAYSO last year please use the username (email) and password which you used in creating an eAYSO account. Note: It is possible that this could be different from the email address you currently use. You will discover that all of the information you entered last year is still there….just log on and open the Myeayso windowshade. Go to the parent section and update all of your information for both parents, then follow the steps to update or add any players and print their forms. 5. You will need to print 3 copies of the “Player Pre-Registration Form” for each player. 6. Write a check.  The Registration fee is $115 per player (to SM AYSO).  This will not be cashed until your child is placed on a team.  Write ONE CHECK PER PLAYER. 7. Make a photocopy of your child's age verification if your child is new to AYSO in San Mateo (birth certificate, passport, insurance card - if birthdate is listed).  Do not send an original document - this will not be returned. 8. Mail the following player registration materials to the address listed below. Three copies of the player pre-registration form A check for $115 (one check for each player) Photocopy of age verification (if new to San Mateo AYSO) Mail these to: SM AYSO Late Registration c/o Jennifer Trizuto 1674 Taylor Street San Mateo 94403 9. If you wish to be contacted when your late registration information is received, please contact Jennifer Trizuto, our Late Registration Coordinator, by email with your request.  Remember to include your child's name and age in your message. NOTE: Incomplete or unsigned registration packets will be returned.
